Please use the following form for registering several employees for the Zoom Sessions.  Visit here for a PDF to mail in with a check.

For best results, please note the following:

1. Fill in each box (use a zero if no TDLR number).

2. Do not rely on autofill for any numbers – TDLR number, credit card number, etc.

3. The form will time out so please complete registration as quickly as possible.

4. Maximum of 8 registrations per charge.
